Validates and commits a form update against authoritative state.
Parameters
- snapshot: FormSessionSnapshot
  The current immutable session snapshot.
- pages: readonly PagePlacement[]
  The pages to process, in document order.
- request: FormOperationRequest
  The request value (FormOperationRequest).
Returns { committed: CommittedFormOperation; snapshot: FormSessionSnapshot }
The updated result.

Throws
FormProtocolError for a stale revision or unknown source document.
